The first 2 I can see fairly readily, as the area under the dome is a
managed environment, the latter is a bit of a push.... But that might
just be me.

Well, as a ball park figure of a mile highest point the diameter should
be some where in the 2 to 20 mile range for a single dome.

There isn't one.

There isn't a real max, though the Millennium Dome in London is
listed as
the worlds largest covering 20 some odd acres. With a 365m diameter.
It really depends if it has to be a strict dome or could it be an
light permiable
structure.
