Granted, I've only read "The Winds of Gath" thus far (although I
intend to buy the rest, as finances allow).  But I didn't get that at
all.  Low passage shouldn't be taken by *unhealthy* folks - these
would die - but a reasonably healthy person wasn't taking very much
risk.  It was simply the cheapest way to travel.  The "low lottery"
which shows up in this book - run by the merchant who owns/runs the
only real settlement on Gath - is arranged to *give* one lucky
Traveller a Low Passage ticket as their prize.
