Presuming a right handed operator.

Presuming the four barrels rotate to the right,

load at bottom, rotate up to the left, 

rotate to the top & fire.

rotate right, eject casing (away from said right handed operator).

rotate to the bottom, load.

Repeat as mood dictates.
